Vitelize Health Inc. provides non-clinical services; independent clinical providers and pharmacies have their own records and privacy notices.
1. Categories, sources and purposes
Depending on your interaction, we collect identifiers and contact details; business and professional information supplied in partner applications; commercial information about requested services and transactions; technical internet/device activity; and state, city and address information you provide. Partner applications may include a business logo or featured coach name. Clinical-service coordination may involve health-related information provided by you or the relevant provider, subject to the health-data notice.
Sources include you, your browser and service providers assisting with the requested services. Purposes include responding to inquiries, partner review, page creation, agreement records, service coordination, transactions, security and legal obligations. Our partner and general contact forms do not request government identifiers, biometric identifiers, medical records or payment-card numbers.
2. Disclosure, sale and sharing
We disclose information as needed to hosting/storage vendors, inquiry-delivery services, payment processors and relevant clinical/pharmacy providers for their roles. Approved partner branding and page content are public; private application and agreement records are not. We do not sell personal information for money.
On the three partner acquisition pages described in our Privacy Statement, Meta Pixel may share technical identifiers and internet activity for advertising measurement. This may qualify as sale or sharing under California law. Names, email addresses, phone numbers and form contents are not included in our Pixel events. We do not install this Pixel on patient, intake, program, eligibility or gym pages.
3. Sensitive information
Any sensitive information handled in connection with requested services is subject to the applicable privacy notice and provider controls. The website does not send clinical information in its Meta events. The partner signup flow does not request patient information or banking details. Contact us to request limits on sensitive-information use where applicable.
4. Your rights
Subject to applicable law and exceptions, you may request to know or access the personal information we hold, correct inaccurate information, request deletion, opt out of sale or sharing, and limit certain sensitive-information uses. We will not discriminate against you for exercising your rights.
Do Not Sell or Share My Personal Information / Your Privacy Choices. We honor Global Privacy Control for website advertising tracking on the browser where the signal is sent.
5. Requests and authorized agents

We process requests within the timelines required by applicable law and will explain if an exception or extension applies. If we deny a request, contact us with “Data Rights Request Appeal” in the subject. You may contact the California Privacy Protection Agency or California Attorney General.
6. Retention
We retain information as needed for the stated purposes, relationship, agreement records, security, legal obligations and dispute resolution. Applicable provider policies govern clinical records.
